US District Judge Brendan Hurson in Baltimore, a Biden appointee, issued a preliminary injunction preventing enforcement while the case proceeds.
The administration is expected to appeal, but the ruling keeps funding intact for now.
Following Trump's order, several hospitals suspended gender-affirming care for minors to avoid losing federal support.
A separate judge in Seattle has blocked the orders in four states in another lawsuit led by Democratic-run states.
